1. A dental and medical history.
2. A digital (low dose) thorough x-ray examination (as indicated).
3. Impressions for study models (as indicated).
4. Intra and Extra oral dental photography (as indicated).
5. A soft tissue oral cancer screening examination.
6. A functional temperomandibular (TMJ) screening examination.
7. A salivary gland screening examination.
8. A periodontal disease screening examination.
9. An occlusal (bite relationship) evaluation.
10. A dental decay screening examination.
11. A cosmetic smile assessment as to tooth size, shape, contour, biting plane, overlap, and the relationship to the opposing arch.
12. An assessment of missing teeth (if any).
13. An assessment of special comfort needs ie. special sedation and pain control modalities.
